Do I have to pay for cable, internet or phone?

Every room has basic cable (61channels) provided.  There are no options for pay channels.

Each of the rooms is wired with two Ethernet connections.  There are no additional fees for Internet access.

Phone jacks are in each room.  However, you are responsible for phone activation fees and monthly charges through Southwestern Bell. (the brick units do not have phone jacks.)  Most students prefer to arrange for a cell phone through a local provider

Yes, but you will both need to indicate on the Room Reservation Form who you want as your roommate.  Choice of roommate is not guaranteed, but those who turn in their paperwork early should get their roommate of choice.
