Transcripts

Transcript Information

A transcript is a certified, official record of a student’s academic progress. It contains confidential information and cannot be released to anyone without the student’s written permission and signature.

Transcripts for a student’s secondary and previous college education, which have been submitted to Independence Community College as a requirement for admission, become a part of the Registrar’s official file. ICC does not reissue or certify copies of transcripts from other institutions. Transcripts must be ordered directly from the institution where the coursework was taken.

Transcript

First two transcripts are free, after that each transcript costs $5.00 and must be paid in advance by cash, check or money order (payable to ICC), or Credit Card (MasterCard, VISA, or Discover accepted).  All fees are payable in advance. DO NOT INCLUDE CREDIT/DEBIT CARD INFORMATION ON YOUR TRANSCRIPT REQUEST. Doing so will result in the request being destroyed and not processed.

How to Order a Transcript

NOTE: Transcript requests will be processed within one week of receipt. However, a longer period of time may be required for processing at the end of each semester and during peak enrollment periods.

A. By MAIL or FAX — send your written request to the following address:

** (You may call 800-842-6063 ext 5430, or 620-332-5430 after faxing your request to be sure that it was received in a readable form and that there are no HOLDS on your transcript.  If you reach voicemail, leave a message stating that you have faxed a request and want to be notified if it was NOT received.  We will call you back if we HAVE NOT received your request.)

Transcript Request Form:

You may use the online form here or include the following information in your written request:

  1. Your current name, plus any other name(s) you may have used while attending ICC, as well as since then.

  2. Your social security number.

  3. Your date of birth.

  4. Your approximate dates of attendance at ICC (include any degree and date earned).

  5. The number of transcripts you are ordering.

  6. The complete address(es) where each transcript is to be sent.

  7. If separate sealed envelope is required, please indicate so.

  8. Enclose $5.00 for each transcript requested (and $5.00 for each faxed transcript requested). Payment must be made in U.S. dollars (cash) or check/money order on an U.S. bank., or Payment by credit/debit card (MasterCard, VISA, or Discover accepted) may be accomplished ONLY by calling the ICC Business Office at 620.332.5416.  This type of payment will no longer be accepted on the request, by email, or by calling any other office. If you include credit/debit card information on your request, it will be destroyed and not processed.  

  9. Your original signature.

  10. Your current address and daytime telephone number.

  11. Indicate if you want transcripts sent after current semester grades are posted and/or after degree statement is on your record.

B. IN PERSON — Complete a request form at the Welcome Desk in the Student Union, Independence Community College, College Ave. & Brookside Dr., KS.  Our office hours are Monday through Friday, 8:00 a.m. — 5:00 p.m.  Please have your student ID or driver’s license available for identification purposes. Please be aware that transcripts requested in person may not be processed immediately.

Other Important Information

  1. No one else (including your parent, spouse, etc.) may pick up your transcript without your written permission and signed authorization. Please identify in your request the name of the person who is authorized to obtain your transcript.

  2. Transcripts will not be issued if any financial obligations exist with ICC.

  3. Some institutions will not accept a transcript directly from the student.

  4. ICC does not accept transcript requests by telephone or email. A student signature is required to obtain a transcript.

  5. Transcripts that are faxed to another institution are considered to be an advising document (unofficial) ONLY, and an official transcript may need to be sent in addition.
